Our work at Sunrise is focused on supporting people to be discharged from hospital, offering them person centred support whilst they can be assessed within the community.
We support people to regain skills, make connections and promote independence. We use an enabling and empowering approach, with the goal of people doing as much as possible for themselves.
We work with people requiring support to set outcomes and goals which are reviewed regularly.
Co-production is central to what we do, we support people to be involved in all aspects of their life through inclusion and engagement.

N had showed significant improvement within our residential service and was stepped-down to one of our supported living homes.
